The Social- and Health section of Lemvig Municipality and the Hague University of Applied Science aim to improve local and European knowledge in health prevention by combating loneliness and social isolation for citizens that are 65+ years of age living in areas of low income and low educational levels. The project seeks to foster inclusion and equal access for these target groups, particularly those with fewer economic resources, whose isolation puts them at risk of developing health problems.

Use HHS research and knowledge in citizen science to better understand the needs and barriers of economically disadvantaged citizens at risk of loneliness living in semi-rural areas Capacity build health and care workers, civil society, and GPs in the Dutch methodology Engage stakeholders in co-creating a local preventive model against loneliness HHS will draw inspiration from Lemvig’s age-friendly community approach Create strategic guidelines on how to approach and dialog with at risk citizens

Increased knowledge of both organisations including health and social workers, civil society and GPs who enhance their capacity in reaching the intended target groups. Improved skills in applying methodologies to reduce isolation and support at-risk individuals. Validation of the Dutch methodology with feedback from community participants, creating a baseline to initiate a follow-on action.
