Objectives

The RE@HOME project promotes inclusive, person-centered elderly care, addressing the challenges seniors face when transitioning into care. Many struggle to accept this move, while families experience guilt and uncertainty. By creating a scalable care model: digital app, VET training toolkit, and public toolkit, RE@HOME ensures dignity, comfort, and continuity of care for seniors, peace of mind for families, and essential support for caregivers, creating a smoother, more compassionate transition.

Activities

The project will develop the RE@HOME approach through co-creation workshops, digital app development, training content creation, and piloting with care professionals, family caregivers, and seniors. Outreach activities will engage care institutions, ensuring wide adoption. An open-access digital platform will host all resources. The final phase includes impact valorisation, targeted promotion, and sustainability planning to ensure long-term use and expansion across Europe in 24 EU languages.

Impact

RE@HOME will produce a digital app to support seamless care transitions, a VET training toolkit to upskill health/social care professionals, and a public toolkit to guide care institutions in applying the methodology effectively. Outcomes include greater dignity and comfort for seniors, reduced family distress, and improved professional caregiving. By strengthening care coordination and emotional support, the project fosters a more inclusive, compassionate and sustainable elderly care ecosystem.
