▶Objectives
The project aims to promote independent living and community engagement for seniors 65+ post-rehabilitation, improving their quality of life and reducing health interventions. Key objectives include addressing barriers to community integration, exchanging best practices, promoting physical and mental health, decreasing loneliness and preventing readmission. The project will provide tools such as an online educational brochure and videos in multiple languages to guide seniors in reconnecting.

This project involves following activities; Identifying barriers for community integration for 65+. Creation of a mapping to help the target group find local activities. Creation of an online educational brochure with guidance regarding community integration. Creation of motivational videos with relatable stories. Project management coordinated by DGI Vestjylland for smooth execution. Meetings, dissemination & evaluation -Transnational and online meetings for knowledge exchange.

The project supports community integration for seniors 65+ post-rehabilitation. Key outcomes include: Barrier Identification & Mapping: Identifying local barriers and mapping activities for the target group for an online brochure. Educational Tools: A multilingual online educational brochure and videos offering practical information. Impact Evaluation: Assessing engagement and feedback to measure success. Dissemination: Sharing results via digital tools, social media and EU platforms.
