▶Objectives
Our goal is to disseminate to patients with Chronic Kidney Disease (CKD) and their caregivers that Conservative Care should be initiated alongside a diagnosis of a serious chronic or life-threatening illness to address the holistic care needs of patients and families, allow adequate care planning, facilitate treatment choices, advance goals of care, and reduce distress among patients and families. Patients should play an active role in decision-making about their treatment options.

The activities are described below: 1. Kick-off meeting in Aegina Island, GREECE (in person) 2. Two transnational meetings: 1 TM virtual and the final one in Antalya (TURKEY) in person 3. Focus groups 4. Webinars using the ZOOM platform with speakers that are involved in Conservative Care (Nephrologists, Nurses, Psychiatrists) 5. Two LLT events in Greece (blended) 6. Disseminate the good practices guide to other European entities that offer support to kidney patients and their caregivers
▶Impact
Our project's likely outcomes: - Draft a good practice guide for all those interested in the field of Conservative Care (CC) for patients with CKD - Create a website where all stakeholders may have access to useful content about CC - Provide training content for different audiences: patients, carers, professionals, social healthcare systems planners, and the general public where they can be informed about the benefits and the quality that CC provides to patients and their caregivers.
